How to switch

Community First Choice (CFC)/Community Options (CO) Programs

During 2014 the Maryland Department of Health and Mental Hygiene (DHMH) adopted a new model for ambulatory programs Community First Choice (CFC)/Community Options (CO).  All participants of ambulatory programs previously known as Waiver for Older Adults and Medical Assistance Personal Care (MAPC) will be receiving service under the new Programs CFC/CO.

In June this year (2015) it was announced that as of October 1, 2015, programs will change to an agency-only model.  The programs will no longer pay to individual independent providers (working under their individual provider numbers).  In order to continue working for the program, participants (and caregivers) must work for an agency.

The participants who already have a MAPC Program do not have to wait any longer.
